What is the health services portal?

The health services portal is a secure system that can be used to schedule and/or cancel appointments, send secure messages to and receive secure messages from your health care provider, view lab results, as well as enter and view your immunization information.

How do I log into the health services portal?

Students can log into the health services portal by logging into their Navigator account, choosing the campus life section, and then choosing the health services portal. Students will then log in to the health services portal by using their student ID # with the "S" in front, followed by "@salemstate.edu" and their existing navigator password. If you are having trouble with your login username or password, contact the Salem State ITS department help desk at 978.542.2036.

Please note: Counseling and health services takes student privacy and confidentiality very seriously. Because the log-in information is the same for the health services portal as it is for your Navigator account, help us protect your privacy - please be aware that by sharing your Navigator log-in information with anyone, you are also granting them access to your private health information that is contained in the portal.

I am logging into the health portal for the first time, what are the first steps I should take?

First, make sure that you read the home page of the health services portal. This gives you information about what the health services portal is and what you can use it for.

Second, review your profile and make sure that the information contained there is correct and up-to-date. You can edit your mobile phone number and choose your mobile phone carrier. By choosing your mobile phone carrier you are giving counseling and health services permission to send you text reminders of appointments that you schedule, either through the health services portal or over the phone, with our office. If any part of the information in your profile, other than your cell phone and mobile carrier, is incorrect or you need to update your current address, you can update this information using your student navigator account.

Now that you have completed these first few steps, feel free to look around and explore the health services portal! You can see where and how to schedule or cancel your future appointments with health services, upload images of your insurance card and your immunization documents, how to send messages to your health care providers, view and print your immunization information, and complete any necessary forms.

What information is included in my profile?

In your profile you will see your date of birth, email address, local phone number, mobile phone number, mobile phone carrier, local address, current primary care clinician and your emergency contact. One of the most important pieces of information located here is your current primary care clinician. This is filled in after you have been seen in health services. This clinician is who you should always schedule your appointments with, unless they are unavailable.

How do I schedule an appointment?

When you click on the appointment link, you will be brought to a page with information about what to know when scheduling an appointment. After you have read the information on this page, you will click on “schedule an appointment.” On the page that opens, we ask that you read the information provided to you on this page, select the category that your visit reason falls into and hit continue. You will then be asked various questions to determine if your symptoms qualify as an emergency, and if so, who you should call.

If your symptoms do not qualify as an emergency, you will be directed to a screen showing you the available dates and times for your visit. The provider available for that visit is also available. Please book your appointment with your primary care provider from our office whenever possible.

Pay close attention to the instructions on the screen that appears after you select your appointment date and time. You may need to complete a clinical questionnaire PRIOR to your appointment. Once you click on “Book” you will be brought to any questionnaire that is required for you to complete. You must bring your ClipperCard and health insurance card to all visits. 

We require that you notify us one hour in advance if you must cancel or reschedule your appointment so that we can offer the appointment to another patient. You can cancel your appointment by calling our office or by cancelling the appointment in the health portal.

What type of appointments can and should be scheduled using the health services portal?

Currently, only appointments for health services can be scheduled using the health services portal. 

You can schedule appointments that fall into the following categories for health services:

  • Same day visit
  • Routine health services appointment (not urgent)
  • Lab test review
  • Birth control start
  • Birth control follow-up appointment
  • Reproductive health appointment such as: STD testing, vaginal symptoms, pregnancy testing and sexual concerns.
  • Tobacco cessation
  • Immunizations and/or PPD tests

What type of appointments should not be scheduled using the health services portal?

Currently, only appointments for health services can be scheduled using the health services portal. Counseling appointment scheduling will be available in the future.

**If you are experiencing any of the following conditions or concerns: chest pain, severe shortness of breath, heavy bleeding, allergic reaction, or other possibly life-threating condition, you should contact emergency services immediately. Call 911 if you are off-campus or call 978.542.6111.

What is secure messaging? 

Secure messaging is a safe, confidential way for you to communicate with the medical and billing staff in counseling and health services. Using this tool, you can:

  • Send a message to the medical billing coordinator
  • Update your medical insurance information
  • Request a prescription renewal from a provider
  • Communicate with a clinician for a reason not listed

These messages are delivered to your provider through our electronic medical record system and become part of your medical record.

Important facts about secure messaging:

Secure messaging is where your provider will send you your test results, so do not share your health services portal login information with anyone. 

You should not contact your provider via secure message for any of the following reasons:

  • If you are having severe or life threatening symptoms (call 911 off campus or 978.542.6111 on campus)
  • If your request is time-sensitive

How do I send and receive messages from my health care provider?

To read messages that may have been sent to you from your health care provider, click on “Messages” on the left hand navigation bar of the health services portal. You will see all messages that you have received.

To send a message to your health care provider, select “New Message” from the top of the messages screen. Choose one of the four options regarding what your message is about, or who you would like to send the message to.

You can use this tool to contact CHS for four different reasons:
  • Send a message to the medical billing coordinator – this message type automatically fills in the recipient; you must enter a subject and message, similar to sending an email.
  • Update your medical insurance information – this message type automatically fills in the recipient; you must enter the subject as well as scroll down to enter your insurance information. Fields marked with ** are required.
  • Request a prescription renewal from a provider – this message type does not automatically fill in your recipient for you. You must select the recipient, who should be the provider that wrote the prescription for you. This information should be on the medication package that you were given when your prescription was filled. If you do not have the medication package, or are unsure who your prescriber was, when you choose the “select recipient” option, you are provided with the names of all the clinicians in CHS and if you are assigned a primary care clinician, the provider you are assigned to is designated here. You can click the option labeled, “Click here to select your primary care clinician.”
  • Communicate with a clinician for a reason not listed – this message type does not automatically fill in your recipient for you. You must select the recipient of the intended message. This recipient should generally be your primary care clinician, or the clinician you have seen for the issue you are writing about. When you choose the “select recipient” option, you are provided with the names of all the clinicians in CHS and if you are assigned a primary care clinician, the provider you are assigned to is designated here. You can click the option labeled, “Click here to select your primary care clinician.”

What forms are included in the forms section? Do I need to complete all of them?

This section includes the following forms:

  • Meningitis Waiver Form
  • Tuberculosis Screening Form

What information is included on the Immunizations page?

The Immunizations page includes the dates of the immunizations that you have entered on your Immunizations Entry Page in the forms section. This separate immunization section is where you can print out your immunization record as well as see what immunizations are still necessary for you to receive or submit. For a list of required immunizations, please refer to the Immunization Requirements page.

Using this page to print out your immunizations saves you a trip into the office if these records are needed for work or for continuing your education after Salem State University.

How to upload your immunization records

Visit salemstate.edu/chs to locate the student health portal

Log into the portal using your navigator login

Click on "Immunizations"

Click on "Add Immunization Record"

Choose file then click open

Click on image to make sure the correct file was uploaded. If the file is incorrect, click on remove before saving.

Click on "Save Records"
